The yearly sign-up (YES), step by step

The yearly sign-up (YES) is the patient's yearly confirmation on the PhilHealth portal. Here is the order of steps and what the clinic logs.

What YES is

YES is the patient's yearly confirmation on the PhilHealth portal. It is done by the patient, on PhilHealth's side, once a year. The clinic does not complete it for them. What the clinic does is make sure the patient can reach it, and keep a record that it happened.

02

Step by step

First, the patient has chosen your clinic and has had the check-in face scan. Second, the first visit is finished and the clinic sends the first visit report. Third, once PhilHealth has received that report, the portal opens the patient's yearly sign-up. This order matters: YES opens only after the first visit report is received, so a patient cannot sign up early (PA 2025-0072, PA 2026-0029).

Fourth, the patient signs up on the portal and a scan reference is produced. Fifth, the clinic logs that reference in SugboDoc. Sixth, PhilHealth reviews and accepts the first visit. The yearly sign-up does not block a visit or a report from being sent. It is a step to follow up, not a lock.

03

What SugboDoc shows

The yearly sign-up tracker groups patients into three simple lists: who still needs to sign up, who is waiting on PhilHealth, and who is done. Each patient shows the next step, the date and the reference. Patients whose check-in scan is logged and whose first visit report is sent appear in the sign-up queue, because those are the people the portal can now open.

04

If the portal is down

Portals go down. When that happens, a paper YES can be kept on file instead (PA 2026-0052). Keep the original signed copy, and note in the patient record that the paper version exists. Do not discard it once the portal comes back.

Common slip-ups

The most common problem is simply that nobody followed up. A report went out, the portal opened, and the patient never completed the sign-up. A tracker that shows waiting patients with dates makes that visible. Read the advisories listed below for the current steps, because PhilHealth can change them.
