YAKAP in one paragraph
YAKAP is PhilHealth's primary-care program for registered members. It builds on PhilHealth's Konsulta primary-care package, so many clinics still see the word Konsulta on forms and report formats. Who qualifies, what is covered and what a clinic is paid are set by official PhilHealth issuances. This guide explains the clinic's side of the year, not the rules themselves. For the rules, read the circulars listed below.
Who the clinic serves
A clinic can only serve and report members who chose it first. Members register with a clinic through PhilHealth channels: eGovPH, the Member Portal or their LHIO. Until that happens, the patient is not on your enrolled list and a report cannot be sent for them. Once they have chosen you, the patient appears in your enrolled patients list with their status, package and visit progress.
Facilities that take part are accredited, or applying for accreditation, with PhilHealth. SugboDoc is a software tool. It does not give accreditation and it cannot speed it up.
What the clinic does across the year
The year follows a short handshake between the patient, the clinic and PhilHealth. The patient signs up with your clinic. At the counter, staff do a face-scan ID check on PhilHealth's own page before the first check-up (PC 2025-0017). The doctor sees the patient and the clinic sends a first visit report to PhilHealth (PC 2024-0013).
After PhilHealth receives that report, its portal opens the patient's yearly sign-up, called YES. The patient completes it, and the clinic logs the scan reference (PA 2025-0072, PA 2026-0029). PhilHealth then reviews and accepts the first visit. Only PhilHealth can do that step. Later check-ups are consultations and are reported before the yearly cutoff.
Where clinics lose time
Most delays come from the same few places: a visit that was never sent, a scan reference that is missing, or a yearly sign-up nobody followed up. Each one has a deadline attached, so a small gap can turn into a late payment.
How SugboDoc helps
SugboDoc keeps every step in one place. A month calendar shows unsent visits with a countdown to each cutoff. A gap checker lists what could hold back payment before you send anything. The yearly sign-up tracker shows who is done, who is waiting and who needs a nudge. Staff always review and send deliberately, and PhilHealth still decides acceptance and payment.
